the realization that no one else is going to rescue you is very very difficult. i think most of us at some level want to be taken care of, to have someone make it all better and make the hurt go away. most people want to be nurtured and it's hard to realize that you sometimes have to give yourself the nurturing you want so desperately. and it's hard when you realize that your dream of everything someday being all right, of people changing and making it up to you, isn't going to happen.
don't run away from these feelings. let yourself grieve the loss of these dreams. it's okay to spend some time being sad. find ways to say goodbye to those dreams and think about how your life will change without them there. think of what will be better and what will be worse. write about how it will all change. think about what you will do with the energy that is freed by the death of these dreams.

I think the idea of taking photos of your goals so you have something tangable is an excelent idea
You do know that each time you feel this way it passes but it doesn't make it feel any better at the time.
Maybe having the goals right there where you can see them will make it a little easier to make it through
