I have a few thoughts...hope some of them are helpful.
First, I agree that counseling probably won't help as much if you cannot be honest. It would probably help a lot if you could tell your counselor what you really need help with. You cannot be committed involuntarily to the hospital unless you are a serious risk to yourself. If you are afraid that this might happen, maybe talk to your counselor about the underlying issues you think might be contributing to the SI. For example, you could tell her you are having problems handling your emotions, or if you are feeling a lot of anxiety.
I would also try to see your Pdoc to make sure your meds are still doing their job. Sometimes they will need to be adjusted or changed if they aren't continuing to work or if what you need changes.
Finally, there are lots of good coping skills you can use to start helping yourself. You are using the SI for something right now...it does something that helps you. So if you can find other, healthier ways to do that, you will be able to choose those instead of SI. I will warn you that in my experience, nothing works as quickly as SI for stopping negative emotions. But there are tons of other options that don't carry the same cost (physical or emotional) and can help for longer than SI can. This board is great for finding new options. I would recommend trying a lot of them, and keep trying even if they don't work right away. It will take some time for you to develop your coping skills.
